Privacy Statement

We are concerned about your privacy. Accordingly, IPAT will not give or sell your personal information (name, address, phone number, etc.) to any outside company or person for their use in marketing or solicitation or for any other purpose.

When IPAT asks for customer information, it is done with the goal of improving the relationship between IPAT and you, the customer. IPAT will maintain the confidentiality of your personal information and will use it only to support your customer relationship with us.

IPAT reserves the right to aggregate non-person-specific data obtained from the processing of IPAT products and to utilize these data for research and statistical purposes. IPAT may also disclose such data to third parties with or without reimbursement or notification. Personally identifiable information will not be released for any purpose other than as we may be required and directed to do so by a court of law.

HIPAA Compliance and Privacy

Protecting the privacy of individuals and maintaining the security of personal information are operational priorities for every IPAT employee. Accordingly, as a business associate of covered entities as defined by the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule (45 C.F.R. part 160 and 164), we comply with the Privacy Rule as it pertains to Protected Health Information (PHI).

Protected Health Information includes the transmission, use, storage, and disposal of physical data, including individually identifiable answer sheets and interpretive reports. IPAT has procedures in place to ensure the security of Protected Health Information that is processed through our NetAssess, OnFax, and Email systems.

Specific procedures IPAT follows to protect individually identifiable health information include:

  • When using, disclosing, or requesting Protected Health Information, IPAT limits access to the information to the person(s) required to accomplish the intended purpose of the use, disclosure, or request.
  • As required by the Privacy Rule, all IPAT employees are trained, as necessary and appropriate, to protect PHI while carrying out their functions.
  • To protect the privacy of the PHI of clients, IPAT has in place appropriate administrative, technical, and physical safeguards, in accordance with the Privacy Rule.
  • IPAT applies appropriate sanctions against any staff member who fails to comply with the requirements of the Privacy Rule. These sanctions may not be applied against an individual who is testifying, assisting, or participating in an investigation, compliance review, or other legal proceeding.
  • IPAT will mitigate, to the extent possible, any harmful effect of any use or disclosure of PHI in violation of policies and procedures or the requirements of the Privacy Rule.
